New Patient Information

Thank you for making your first appointment at NIHA!

We welcome you and look forward to helping you achieve a higher level of health through integrative medicine.

Prior to your first appointment:

In order to expedite your first appointment please download  forms  and complete registration and medical/dental history prior to your arrival.  It may take up to an hour to complete these forms, and we  want you to get the maximum benefits from your time here. If the forms are not completed before your appointment, please arrive at least one (1) hour prior to your scheduled appointment. We need these thoroughly completed in order to do a comprehensive evaluation.
We request that you bring with you any recent laboratory tests, medical reports and bottles or samples of all medications, herbs, homeopathics and nutritional supplements [vitamins, etc.] that you are presently taking. This is extremely helpful for your initial evaluation. We encourage you to prepare any written questions ahead of time and bring them to your initial appointment.

Your First Appointment:

We have compassionate patient coordinators assigned to each practitioner to help coordinate your treatment, insurance, and payment. The patient coordinator is your 'contact person' with whom you have made the first appointment. The coordinator will assist you should you have questions before, during, or after your NIHA appointment. 

In order to accommodate our chemically sensitive patients we are a fragrance-free office so we ask that you not wear perfume or scented lotions on your appointment days.

Payment and Insurance:

Payment in full is due at the time of service at NIHA.

We accept  cash,  check, MasterCard, Visa, Discover and Money Orders.
As of September 1, 2008 we will no longer accept American Express  as a form of payment.

We offer our patients comprehensive care which limits the ability to work within the limitations of most contractual agreements for health insurance. If you have insurance, we will provide you with the standard forms needed for you to file an insurance claim. You are responsible for filing your own claim and following up as needed. Please be advised, however, that many of the services provided are not covered by most insurance companies. Please contact your insurance company for information on your coverage. We are not providers for HMO insurance, Medicare, or Medicaid. For additional information please see the Insurance Filing Information section.

From Baltimore & Maryland

EASIEST: Take I-95 South to I-495 West heading toward Rockville.
From I-495 take Exit 34 and stay in the far right lane which forks and loops south onto Wisconsin Ave., Route 355 South. ( Rt. 355 South is Rockville Pike, which turns into Wisconsin Ave.)
Stay on Wisconsin Ave. for about 4 –5 miles, crossing Western Ave. The next light is Jenifer Street. Take a Left on to Jenifer Street. Turn Right down ramp into parking garage (same garage as Elizabeth Arden) The elevators for the building are between the parking garage floors, accessible only via steps. The office is on the 4th floor, suite 401

FASTEST: Take I-95 South to I- 495 West heading toward Rockville.
Stay on I- 495 until Connecticut Ave. (Exit 33) Take Connecticut Ave. South toward Washington, D.C. Go ½ way around Chevy Chase Circle and continue straight on Connecticut Ave.
Turn Right onto Military Rd. and continue for approximately ½ mile
Turn Left onto 43rd St., go to the end of the block where you are forced to turn Right onto Jenifer St. Take an immediate Left down the ramp into the parking garage (same garage as Elizabeth Arden) The elevators for the building are between the parking garage floors, accessible only via steps. From Virginia
From I –95 North take the Capital Beltway ( I- 495 North) toward Tysons Corner and Maryland Take I –495 North into Maryland, take River Road East (exit 39) toward Bethesda, MD, and DC Follow River Road for approximately 4 miles Turn Left at Western Ave. and follow for approximately ¼ mile Turn Right onto Jenifer Street Follow Jenifer Street across Wisconsin Ave Turn Right down ramp into parking garage (same garage as Elizabeth Arden) The elevators for the building are between the parking garage floors, accessible only via steps. Via Metro
Take the Red Line to Friendship Heights Metro Station. Exit the station via the Jenifer Street Exit – via elevator Upon leaving Metro station, 5225 Wisconsin Ave is directly across the street. The office is on the 4th floor, suite 401

Handicap Access
The parking garage has handicap parking spaces near the elevators, but to get to the elevators you must walk up or down 7 steps. If steps are difficult to use, we recommend that you park on Wisconsin Ave. in front of the building or on 43rd Street behind the building and use those handicap accessible entrances to the building. If you need assistance, please advise us in advance and we will provide support and a wheel chair if needed. We apologize for the inconvenience.

Insurance Filing Instructions

Dear Patient:

NIHA does not file or coordinate insurance reimbursements although we do provide information on the services rendered for you to submit. The information below should assist you with filing for insurance reimbursement.

Visit Check-Out-

For Drs. Beals, Garzon, Mines, Rind, and Solomon:
Services are provided on a fee-for-service basis and insurance support will not be provided. A bill describing services will be provided, but will only use procedural codes at the discretion of the practitioner. You may complete your insurance company's claim form and submit a request for coverage, but NIHA  does not provide a claim form (HCFA 1500), nor will it respond to requests for additional information. Patients may request a copy of their records and personally respond to requests by their insurer for additional information. At check-out, you may request a copy of your practitioner’s notes for that date of service. Future requests for another copy will be available at a cost of $25.

For all other NIHA Practitioners:
NIHA will provide claim forms for submission to insurance; submission is the patient's responsibility. Claim submissions may or may not be for covered services and may or may not include procedural codes or other data sufficient to support your insurer's determination as to what services it will reimburse.

For Dental Practitioners:
Patients will receive an ADA (dental claim form), that will include all necessary information that is needed to file for reimbursement, along with a receipt for the services rendered.

Filing for Reimbursement- Methods for filing vary by company, but generally you will need to mail the bill provided at check-out (we suggest keeping a copy for yourself) to the address or post office box provided by the insurer or your employer. All of our practitioners have opted-out of Medicare. For secondary payers to Medicare, we will provide a copy of the practitioner’s opt-out affidavit.

Insurance Requests- If we get requests for additional information, we will forward the request to you along with copies of the requested notes, x-rays, etc. for submission to the insurer. If your insurer requires their form be filled out and submitted, you will need to fill it out and in the section where your practitioner’s signature or specific information is needed write, “see attachment.” Then attach the claim form, notes, x-rays, etc. we provided you and mail them to the insurer. As always, we urge you to make a copy of the information before submitting.

Not Recognized Care- Some of the care provided may not be recognized by your insurance company. Often these claim forms will be returned to you or us with a message like, “in order to process your claim we must have a correct CPT code” or “CPT code not valid”. Some of the services we provide do not have CPT codes, as the codes are set by the conventional medical community. Insurers rarely pay for services without a CPT code. We can provide you with a brief description of these services that you can submit for claim review, in the event you still wish to pursue payment.

Reimbursement Problems- Securing payments for insurance companies can be difficult, whether the services rendered were conventional or alternative. Your best bet is to keep close track of what and when you submit to your insurer and challenge or appeal any decision they make which is not explained to your satisfaction. If you are still dissatisfied and believe the insurer has not properly reimbursed you, you have two other options that are occasionally effective:

1. Contact your employer and ask them to intervene on your behalf. They often have more clout with the insurer, since they hire the insurer and pay the bill.

2. Contact your state’s insurance department. They will typically take your complaint directly back to your insurer, asking them to explain why (based on the contract) that something was not covered.

Insurance Billing Services- There are individuals and companies which, for a fee, can assist you with claims filing and reimbursement from insurance or flexible spending account administrators. A list of firms offering these services is available upon request.
