Thank you for choosing NIHA for your integrative medical and dental care. Please download  patient forms and complete them prior to your arrival.  We take a very thorough history and it may take up to one hour to complete these forms, and we  want you to get the maximum benefit  from your time here.

If forms are not completed before your appointment, please arrive at least one (1) hour prior to your scheduled appointment. These must be completed in order to do a comprehensive evaluation.

Please bring your completed forms and recent medical records, laboratory tests, medical reports, x-ray reports, and bottles or names of all prescription medications, vitamins, minerals, herbs, homeopathics, and other nutritional supplements etc. that you are presently taking. This is extremely helpful for your initial evaluation. We encourage you to prepare any written questions ahead of time and bring them to your initial appointment.

Fragrance Free Office
In order to accommodate our chemically sensitive or allergic patients we kindly ask that you not wear perfume, cologne or scented lotions on your appointment days.

Payment in full is due at the time of service. We accept  cash, check, MasterCard, Visa, Discover and money orders.

Garage Parking and Handicap Access
We provide complimentary 2 hour parking for patients who park in our underground  parking garage. The garage entrance is the first right after turning onto Jenifer Street from Wisconsin Avenue. For handicap access turn right at the second right on Jenifer Street and push the intercom for designated parking and direct elevator access. An attendant will open the door remotely.




Telemedicine Appointments

Telemedicine virtual appointments are offered for current and new patients and offer the same high quality care as an in-office appointment. We are able to provide continuity of care to current patients as well as assess new medical patients from the comfort of home through this online tool.

Call 202-237-7000 to schedule and request a telemedicine appointment. The coordinator will email you a link to use for your online telemedicine appointment with the doctor.


Day of the Appointment

We use, a HIPAA-compliant secure platform which protects your privacy. There is no need to download any software prior to the appointment – just click on the link at the scheduled appointment time.

You can use a smartphone or tablet with camera and microphone, but we recommend a computer for the highest quality. Recommended web browsers are Google Chrome and Mozilla Firefox. Check your audio and video settings before your appointment, and have a good internet connection.

2.  The coordinator will send you a link to get on the Doxy appointment. Click on the link.

3.  Type in your first name and initial of last name, example: Teresa G

4.  Enable camera and audio speakers.

5.  The doctor will start the visit.


After Your Appointment

Once complete, the services rendered during your telemedicine visit will be coded appropriately and insurance claim forms will be mailed to you for submission to your insurance company for reimbursement. 

Telemedicine has recently been approved by most insurance companies. Check with your insurance company to verify you will be reimbursed for a telemedicine visit. 







We offer our patients comprehensive health care which limits the ability to work within the limitations of most contractual agreements for health insurance. We are an out-of-network health provider. You will be responsible for payment at the time of service, after which we will provide you with an insurance claim form; you should have everything that you need to submit your reimbursement claim to your insurance company before you leave our office. 

You are responsible for filing your own claim and following up as needed. Please be advised, however, that many of the services provided may not be covered by most insurance companies. Please contact your insurance company directly for information on your coverage. We are not providers for HMO insurance, Medicare, or Medicaid. The specific information below should assist you with filing for insurance reimbursement.


Our goal in Holistic Family Dentistry is to maximize your out-of-network dental benefits and minimize out-of-pocket expenses. We are an out-of-network provider with ALL insurance companies. As a courtesy, we will file your dental claim form to your insurance company to assist you in getting reimbursement,  and provide a receipt for the services rendered. Payment is due at the time of service.

Faster reimbursement for MetLife and Guardian Insurance: for all out-of-network patients with Metlife and Guardian dental insurances, we are able to expedite dental claims through faster electronic methods and get your return of dental benefits back to you now in approximately 7 days.

We also offer flexible payment options with Care Credit financing, a healthcare credit card company that allows you to defer your out-of-pocket expenses without interest. 



Our goal is to provide you with the highest quality medical care and necessary information for maximizing your insurance benefits and minimizing your out-of-pocket expenses. You will receive a medical claim form that will include all information for you to file for insurance reimbursement, along with a receipt for services rendered. NIHA is an out-of-network provider only and does not participate with insurance companies or other provider networks.

Payment is due at the time of service unless other arrangements have been made in advance. We generally do not accept assignment of insurance payments.

Insurance claim forms and information are provided to patients at the time of visit for submission to your insurance company. While submission of claim forms is the patient's responsibility, you will receive everything that you need to immediately file your claim at the time of check-out.


NIHA reserves the right in preparing claim submissions to include those procedural codes or other data that accurately reflect services, and to invoice non-covered services using internal descriptions without the use of procedural codes. Please understand that your insurance company may deny coverage because of differences between integrative and conventional medicine. Please contact your insurance company directly for information on your coverage. NIHA will make every effort to provide sufficient information to allow an insurer to determine what services it will reimburse, but it is not responsible for any insurance company decision.

Some of our practitioners may provide limited primary care services as defined by mutual written agreement. They are considered out-of-network providers.  You will receive a medical claim form that will include all information for you to file for insurance reimbursement, along with a receipt for services rendered.

Medicare will not cover any services or procedures performed at NIHA, as the doctors have opted out entirely of the Medicare program. Claims cannot be submitted to Medicare and patient is financially responsible for any services or procedures received. However, several of our medical doctors do provide Medicare patients a discount. Please ask for further information at the time you make your appointment. If a patient has secondary insurance, we will provide an opt-out letter to assist in getting coverage.

NIHA is a nonparticipating Tricare provider, and Tricare patients are responsible for payment of NIHA’s full fee schedule at time of service and will receive reimbursement directly from the Tricare carrier for the amounts payable under Tricare’s fee schedule. In order for NIHA to bill its service rates, Tricare patients must review and sign a Request for Non-Covered Services that will give advance notice of anticipated fees.

There are individuals and companies which, for a fee, can assist you with claims filing and reimbursement from insurance or flexible spending account administrators. A list of firms offering these services is available upon request.

Our office is easily accessible by the Washington Metro's Red Line at Friendship Heights. When exiting the station, take the Metro elevators at the Jenifer Street exit. The entrance to our building is between Ferguson  and the Red Door Spa, located directly across from the street when you come off the Metro elevator. Take the building elevator up to the 4th floor and exit to the left to Suite 402.


Our office now has a Capital Bikeshare station located just across the street from our building at Wisconsin Avenue NW.


Our office is in the District of Columbia on Wisconsin Avenue, across from Mazza Gallerie, and close to all points in Maryland (e.g. Bethesda, Rockville, Baltimore) and Northern Virginia.

From Baltimore & Maryland
EASIEST: Take I-95 South to I-495 West heading toward Rockville.
From I-495 take Exit 34 and stay in the far right lane which forks and loops south onto Wisconsin Ave., Route 355 South. ( Rt. 355 South is Rockville Pike, which turns into Wisconsin Ave.)
Stay on Wisconsin Ave. for about 4 –5 miles, crossing Western Ave. The next light is Jenifer Street. Take a Left on to Jenifer Street. Turn Right down the first ramp into Colonial Parking garage. The elevators for the building are between the parking garage floors, accessible only via steps. The office is on the 4th floor, suite 402.

FASTEST: Take I-95 South to I- 495 West heading toward Rockville.
Stay on I- 495 until Connecticut Ave. (Exit 33) Take Connecticut Ave. South toward Washington, D.C. Go ½ way around Chevy Chase Circle and continue straight on Connecticut Ave.
Turn Right onto Military Rd. and continue for approximately ½ mile
Turn Left onto 43rd St., go to the end of the block where you are forced to turn Right onto Jenifer St. Take a Left down the last ramp (before Wisconsin  Ave intersection)  into the Colonial Parking garage. The elevators for the building are between the parking garage floors, accessible only via steps. The office is on the 4th floor, suite 402.

From Virginia
From I –95 North take the Capital Beltway ( I- 495 North) toward Tysons Corner and Maryland. Take I –495 North into Maryland, take River Road East (exit 39) toward Washington DC. Follow River Road for approximately 4 miles. Turn Left at Western Ave. and follow for approximately ¼ mile. Turn Right onto Jenifer Street. Follow Jenifer Street across Wisconsin Ave. and turn right down the first ramp into Colonial Parking garage. The elevators for the building are between the parking garage floors, accessible only via steps. The office is on the 4th floor, suite 402.

Handicap Accessibility

There is street level parking in front of the building on 5225 Wisconsin Ave. with access to the lobby. Or, there is handicap accessible parking on level P-1 of the Colonial Parking garage, with the elevator on the same level. Follow directions above to the Colonial parking garage on Jenifer Street but turn right at the SECOND right for entrance to handicap accessible garage. The P-1 entrance has a closed garage door and does not have an attendant, but has a call box and a camera that the attendant uses to remotely open the P-1 door.
Pull-up to the garage door and push the button on the intercom.
An attendant will either answer or remotely raise the garage door-- there will be NO parking ticket provided and none will be needed, as you will not be charged for parking in P-1 for handicap access. Drive into the garage and select one of the handicap parking spaces. Go to the P-1 elevator, which is on the same level as the parking. Select the "Up" button and NIHA is located on the 4th floor, suite 402. If you need assistance, please advise us in advance and we will provide support and a wheel chair if needed.
